LBBB, RBBB PVCS

by lak, Jul 06, 2007 12:00AM
I am 49 years old with a viral cardiomyopathy that started last fall and I am on Coreg 12.5 for that.  I also have a LBBB and many pvcs.  I arrived at my GP office feeling very bad. Extremly fatigued, dizzy , palps and very well.  She did an ekg and called a Ambulance stat.  She said there were no reg beats.
Went to hospital given magnesium and potassium and cardio basically just dismissed it said small leaky value and low mag and potas caused it
When I went back to the enternist she and I looked at the strip from ekg.  She said I also had a RBBB.
She is doing extensive testing and now has orderd a second 24 urine culture and a serum chromogranin.
I also have an appt with an EP who cannot get me in July 24th.  I am still home and have taken short term disabilty due to all of my symptoms snf excessive fatigue.
Can the fatique be caued my the electrical systom in the heart?
Can you have a LBBB RBBB with out complete block?
Do you have any idea what she could be looking for with the urine test?

I am not sure what she is checking for with a urine test, but the only urine test that I check in  the setting of a cardiomyopathy is for a  pheochromocytoma. In this case would check a serum and urinary levels of catecholamines.  The EKG showing a left bundle-branch block and/or a right bundle branch block probably means that your heart is dilated and severely dysfunctional and this is probably causing your fatigue and inability to carry on normal activities of daily living.  In other words, you probably have low output congestive heart failure and probably should see a cardiologist that specializes in this condition.  Generally EP practitioners are specialists that focused on the placement of defibrillators and pacemaker, and this may be all you get out of the appointment on July 24. You can have alternating left and right bundle-branch blocks without actually having complete heart block which he cannot have both of them locked at the same time without having complete heart block.

I asked the question RBBB & LBBB before.  I'm a bit different to yours.  My cardio said I didn't have PAC or PVC at all.  All I have is wpw???  Do you feel chest tightness like an elephant sit on it?   I do but they thought is my indigestion or heart burn.

My pacemaker didn't seem can pace out of LBBB.  It caused LBBB and make me so unwell.  I don't know why the dr would say it helps????  Also, my pacer unable to detect my LBBB or events.  Every time my cardio said it didn't record any event, just a few skip beats.  Well, my 24 hours holter said a lot of events then he said it didn't make sense.  I give up of hopes and decided live with a miserable life!  I'm crying why I should listen to them and put a pacemaker there!!!!!
