Left sided chest and arm pain

I am 35 yrs old, have a history of very high cholesterol since I was 17.  Smoker of 25 yrs. My mother has had 3 heart attacks her first was at the age of 45.  I also have an aunt on my mother's side that has had 2 MI's and another grandfather that has had numerous MI's. All EKG's, chest x-ray's, and labs were normal with her first heart attack. My grandfather died of atherosclerosis at 56.  For the last year I have been having on and off left sided chest pain with pain radiating down my left arm.  Some burning in the chest wall as well.  No sweating, some nausea, lots of fatigue.  I have GERD that I am being treated for.  I have been seen in the ER on numerous occasions over the last year and all EKG's, blood work, and chest x-ray's are negative. For the last week and a half I have had constant chest and arm pain that is tollerable.  I was seen again in the ER this past week with the same results everything was negative and no answer as to what was causing the chest pains.  They do get worse with physical exercise and stress.  I feel like the ER is blowing me off because I have been in so many times complaining of chest pains.  I feel like they think I am one of their regulars.  I do have a nursing background and have know what the ER thinks of their "regulars".  My concern is that I may have a blockage that is causing this pain and that I am just sitting around waiting to have a heart attack.  I am somewhat limited to what I can do because I have no insurance.  Any advice you can give me would be truly appreciated.

You need to see a cardiologist and get a stress test. You can apply for medicaid and have a doctor recommend you a good cardiologist in your area. Or you can play it up real good at a different hospital that has a cardiac center and they can give the stress test to you. You have a family history of heart disease and your presenting with arm and chest pain..add those together and you definitely need a stress test with the dye so they can see if you have any significant blockages or not.
