If anyone can help with this case detailed below I would be very greatfull!

40 Year Old Patient with paternal exposure to Radiation during H Bomb Tests.

Born with Congenital Bells Palsy
Unfused Neural Arch
Blind in Right Eye
Secundum Arterial Septal Defect (LARGE)
Hemi Anomalous Pulmonary Venus Drainage
Enlarged Heart – Successful Surgery 1971

Sinus Vernosus defect of lower atrial septum (12mm diameter) –Surgery Successful 1991

Cateract Removed at age 18 & 35
The above patient was relatively healthy despite the medical history until age 36. Since then there has been a marked deterioration in general health, with significant degeneration of existing congenital abnormalities.

1. 3rd Cateract forming in Right Eye
2. Increased breathlessness, in conjunction with severe episodes of SVT;s & Atrial Fibliration
3. Episodes of SVT’s treated with Cardiac Ablation in 2006.
4. Minor Inferiour MI in Jan 2007
5. Neural Arch degeneration with stenosis and neurological symptoms including bladder control issues, loss of power to the legs.
6. Increasing episodes of breathlessness
7. Aged in appearance significasntly
8. Enlarged Heart
9. Moderate Ischemia to the Basal Wall captured during chest pains.
10. Right Bundle Branch Block
11. Frequent Episodes of central crushing chest pain radiating to the Left Arm (tropinin test show 0.01-0.07). Arteries clear. Pain is debilitating, consciousness has been lost on 3 occasions. ECG’s during pain show Accelerated Junctional Rhythm, Heart Block from 1st Degree to AV Diss. Numerous Significant Arrhythmias.
12. Regular SVT’s , Dropped Beats, Ectopic Beats, Couplets in tracing.
13. General Deterioration of health and well being.
14. Prinzmetals Diagnosed but non responsive to treatment and Calcium Channel Blockers aggravate rhythms.
15. Pale in appearance, hair completely white.
Local Cardiologists are at a loss and offering only palative treatments. We would welcome feedback, suggestions and any help possible.

I would probably focus on the SVT and the heart rhythm at first, it sound like this is an ongoing problem and it should be dealt with at a tertiary care center by specialists who deal with this on a regular basis.  It could certainly be dealt with here at the cleveland clinic.  The chest pain is not related to CAD, and this is a good sign.  Medical therapy is typically very effective and calcium channel blockers arent' the only ones that could be tried.  Again, I would also recommend dealing with this at a similarly specialized care center.
