I am 54 and newly found to have "mild Tricuspid regurgitation with an estimated PA systolic pressure of 30mmHg". The echo was essesntially normal with LV wall motion and ejection fraction normal at 60%, Mild TR, No pericardial effusion or thrombus seen.   My question is how do I keep this from progressing? Right now I have no hypertrophy of any of the chambers. The PA systolic pressure seems borderline high to me. I weigh about 97-98 lbs, teach aerobic water and land exercise every day--all out for approx 1.5-2 hours, distance bike, etc. I work as a nurse and nursing home administrator so I am on the move. The only symptoms I have are occasional palpitations at rest and orthostatic hypotension if I rise too quickly. I have low BP and low HR and seem to always be dehydrated from exercise. I experience no symptoms while exercising. My MD recommendation was to repeat the echo in 2 years. Is that too long? What can I do? I see right-sided failure down the road....

I don'tt think that this is an abnormal finding in someone with a high degree of fitness. It is probably related to your increased cardiac output resulting from an increased stroke volume and not from increased pulse rate. I would continue to exercise aerobically, but avoid extreme sports such as Ironamns, marathons and the like as these are too extreme to be considered heart healthy, and have been shown to cause heart damage. In other words work out aerobically but not to excess, where dehydration and increased cathecholamine levels can cause worsening pulmonary hypertension and cardiac damage.
