: I was diagnosed with cardiomyopathy at two months post-partum.  I was originally at severe dysfunction in May '97, but have improved with medication and rest.  However, over the months since diagnosis I have experienced serious changes in symptoms over each menstrual cycle.  In the last week of a cycle, I become gradually weaker and shorter of breath, have slower heart rate and lower BP.  Shortness of breath or exertion brings on chest pain.
My question is: Has there been research to study the effect of female hormones on heart muscle condition and cardiomyopathy?  I have described this cyclic effect to several very good heart failure specialists and do not feel I'm getting enough help in this matter.  I wonder if hormones caused my heart weakening and if this is preventing further improvement.  It just seems that, since pregnancy is known to worsen or cause cardiomyopathy, the hormonal link would have been studied thoroughly.  However, no one has offered any advice other than to suggest my experience is linked to an anxiety brought on in premenstrual days.
I will appreciate any suggestions or referrals you can offer.

Dear Madelyn,
As you pointed out, the cause for peripartum cardiomyopathy is largely unknown. It is thought to be from immunological changes during pregnancy. There has been some association with a hormone, prolactin (a hormone impotant for lactation). However, this does not seem to play a very important role. The symptoms that you feel with your menstrual cycle could be from water retention with hormonal changes. I am not aware of any hormonal therapy that is beneficial in peripartum cardiomyopathy.
If you are seeking a second opinion or further evaluation, an appointment with one of our heart failure cardiologists can be arranged by calling the Cleveland Clinic at (216) 444-6697.
