Myocardial ischemia - Silent heart attack

Back in March of 2003 I went to the ER with chest pains, SOB, my BP was 120/90  75 BPM for those how know how to do the math...I will throw this in: PR INT 124 ms; QRS dur 72 ms ; QT/QTc 412/450 ms PRT AXIES  53 51 36 normal ECG and normal sinus rhythm

(normal BP is 103/63 on an average)

they sent me home, less than 2 weeks later I went back to them again with the same symptoms
this 2nd ER TRIP revealed this:
BP  92/68 closer to normal for me.. 76 BPM
PR INT 116 ms; QRS dur 70 ms ; QT/QTc 400/446 ms PRT AXIES  46 67 37
impression: loss of R wave & Deeper Q in V3 ... poss old septal MI..
SO ok I had a mycardial Infarction..
although they sent me home again they called the next day to tell me they found fluid around my lungs and heart and wanted me to follow up with a cardiologist ASAP which I did and that is another story.. and another and another.. but to finish  the cardiologist ran a stress test and i went back to discuss that with him and some newly graduated resident reviewed my results shuckled it off *** no big deal and said to make sure you dont fall off the face of the eartth we will see you in a year.. I never went back.. I moved to Fl the follwing year and then the stories got worse..

Okay well time has passed I have had several 24 halters,  echos, and ekg's preformed since this and they have found that I still have an ECG indicating an old MI..
along with having a halter that shows on 81696 beats during a 24 hour period:
my average heart rate being 102 BPM  ranging from 72 to 154 during testing.. and when i had chest pains the rate was picked up at 109 and 103 BPM during those points..tachycadis of 173  highest rate of 154  bpm  for a period of 01:06:08 and the heart rate was above 100 BPM for eight hours and 13 minutes out of a 24 period. ST 38   highest 141 BPM for a period of  00:20:20
prior to these testings but after the incident in 2003 : i was admitted to the ER in 2005 with SOB and chest pains my BP was 188/115 they kept me over night for observation nitro brought my symptoms under control and then ordered me to follow up with a IM physician who told me to stop the nitro, aspirin, lasix and toprol the hospital had placed me on. 3 months later I returned to the local walk in clinic and the MD there was upset and told me to stay on the meds the hospital follow up physician told me to stop taking. So after all this confussion and a few years later.
to summerize my overall health
i have COPD, and HDL risk ractor of between 5.1 to  5.9 on average since age 32 I am presently age 49 nicotine 2.5  mcg/ml.. I have chronic back pain or over 10 years due to DDD..
I lost my  echo cardiogram from 2006 but that showed some issues as well something about a tricuspid regirgitation and I can not recall the other issues..
My question is this:
I went to my general told him my chest pains have returned he put me on a 24 hour halter again.. said my ST were 2.5 for moe than a minute.. and I had again the rapid heart rate which he was expecting and not worried about it so much.. but the ST depessions had him concerned.. so the next day a cardiologist was calling to tell me I had an appointment today.. I went to that and the cardiologist is asking me if I ever had a carotid artery test? I said no.. since i never had a full blown heart attack no one caught me while it was happening.. although it happened within a 2 week period.. probably later that night that they sent me home.. and then again some where when i was in FL during that hospital stay something happened again I think but whatever..
WHy has no one ever run any other tests outside of EKG's ECHO's and the standard of tests to find out why I have an abnormal ekg? ok so we know i have rapid heart rate  and my ST depressions are prelivent.. he asked me if i was on cholestral medication or BP medication I told him what i was on and he said there is a reason why you are having chest pains and why you are having ST depressions and no one has been treating you for them or bothered to find out why? he was totally amazed.. I am also : that is my question I think..

How can I have gotten the past almost 10 years still feeling the tighness in my throat.. sharp pains in my left sholder and jaw and yet only be on toporal and lasix for my heart? yet i have been seen by at least 6 different doctors in 2 different states and no one thought to run any of these tests and are they neccessary? or is this senseless.. am I taking enough medication? and if I am why am I still having this chest pains and they are getting worse lately.. is there anything I can add to this to help clarify anything am I missing something? I am obviously new to this or I would know what is wrong with what i have been told up to this point. I didnt think to much of all of it.. cause the one doctor in FL said I didnt need all of those meds.. so either I do or I dont.. how can one doctor say I do and another one say I dont? and why are all of my EKG's abnormal now saying 'OLD Septal MI"... if there is nothing wrong? I am upset now  cause for years I thought very little of it . but now looking over my records.. I see they are not normal.. and i still have pains that are getting worse..  please anyone want to help guide me to get the right help here I would appreciate it.. I know my family history is horrible on both sides.. but my mom cant help me either.. she goes with me so i dont forget what is said cause my memory is horrible unless i have a record of what was said or done.. please help me understand I dont know how to read an EKG seems very confusing.. but it says on the thing OLD SEPTAL MI what is septal.. sounds sick...

Consider Anteroseptal Infarction  Q/S in V1 & V2 yet the other EKG says V2 and V3 wow why are they all slightly different? What is wrong with my heart?
