New Echo findings

I am 53 years old. I have a Bicuspid Aortic Valve, and a small Diverticulum in the inferior basilar wall.  My recent echo shows the following:  Miltral Valve is moderately thickened., Mild MV Insufficiency, Moderate Tricuspid Valve Insufficiency, Mild Pulmonary Hypertension, Moderate Aortic Valve Insufficiency.

I have always had a "mild" leak in my aortic valve but none of the other valves.  And my "mild" leak in my Aortic valve is now "moderate" I also have PVC & PAC Arrhythmia's, which the doc said not to worry about. Does anyone know what I might be looking at in the near future?  Such as surgery?  Or do you think they will just continue to monitor me?

With aortic stenosis or insufficiency associated with a bicuspid valve, the most important thing to look for is symptoms.  If over time you notice that your exercise capacity has gone down, or is going down, more than what you'd expect based on your age, this is the time to get ready for valve surgery.  At this point they will continue to follow you clinically.  They will also continue to get yearly echo's to see how the vantricular dementions are.  With Aortic insufficinecy, the ventricle can become to fails slightly before the development of symptoms, and this is also the right time to think about surgery as well.  As far as the other valves, they will continue to watch them as well.  As long as they remain mild nothing will be done, but if they progress over time, you should expect having them repaired as well.  The PVC's are probably related to the aortic insufficiency, but one thing to keep in mind is that they also be related to the diverticulum.  When it's time to fix the valve they should also fix the diverticulum.
