Ongoing undiagnosed chest pain

I'm a 41 year old man, 5' 6" 225 pounds, IT worker.  I don't smoke but I do sometimes drink too much.  My diet was not very good (obviously at 225lbs) but I have actually lost 20 to 25 lbs and I am now eating quite healthily.  There is a history of heart disease in my family and both my total cholesterol is too high and my HDL/LDL is less than 1.  I'm taking metoprolol for a fast heart rate which was causing high blood pressure.  I've gone to out-patients/ER several times when I've had this high heart rate and at other times when I've felt a pinching pain where my pectoral muscle connects to my sternum.  The ER staff have done ECGs and blood work but have found nothing.  I also had a stress test performed which was reviewed by a cardiologist but I am concerned my heart rate was not high enough because of the metoprolol.

Because of exercise and an improved diet my blood pressure and heart rate seem normal now and I take 25mg of metoprolol, down from 100mg.  The cardiologist said no issues were apparent.  But I still frequently feel this pinching sensation in my chest around my sternum.  It's never been accompanied by shortness of breath or dizziness.  As for it radiating into my arm I'm not sure because I have a shoulder issue (bursitis).  I'm not sure how to proceed.  I am continuing to work at losing weight by eating healthily and I am exercising though not as consistently as I need to.  Strangely I never notice any issues when I am exercising but usually at rest.  Last evening a pain in my chest woke me from my sleep but went away almost right away.  I would like an echo cardiogram and an angiogram dye test but no doctors seem to want to go this route.  I normally don't visit the doctor's office much but this seems to be a chronic issue for me over the last 3 to 5 years.

I wasn't going to answer this post until you said it never bothers you at exercise, but will wake you from sleep. I do have heart disease, have had a couple of heart attacks and have stents in my heart and elsewhere.  I had chest pain ever since the first cardiac stent.  I went to the ER on several occasions with the chest pains and because of the heart disease I was always admitted and had test run, only to come out fairly normal. My main complaint was that I had these chest pains wake me from a dead sleep and I would have to tak a Nitro pill to relieve it. April of 2010 I went to the ER because I was having these chest pains about 12 to 15 times a day and they would wake me.  In the ER I had a severe chest pain (they always felt like the chest pain I had with my heart attacks) and the monitor was going crazy. One nurse seemed concerned, the other not so much.  The tests were negative for a heart attack, but again, because of the history of heart disease I was admitted to the ICU and a angiogram was ordered for the next morning. While in the ICU I continued to have the chest pain and the monitor would go crazy and start beeping. I told the nurse and she told me it "was junk" because I had moved. I knew that was wrong. After the 3rd time I was able to convince her of this, and she paid more attention to the data that would print out at her station.  She was able to see that I was having vasospasms.  These are harder to diagnose. I do not know why, but the spasms take some real effort on the patient's part to convince the medical personnel. I have said over and over again, I could walk 3 miles briskly and not have any chest pain, but let me go to sleep and here they would start. Another thing to consider is unstable angina.
