POST BYPASS surgery and Angoigram

Hello Dr.  My dad had undergone a  bypass surgery on 27.04.10. he had 3 blockages. main arteries were blocked. after the by pass surgery within two months time we got a ct angio done in the month of august and the blockages reappeared, recently last month we got an angiogram done on 14.06.11. I am giving you the details of the final  report and want your help and wants to know what has been revealed. Is the situation very critical. what do i do next.
FINAL DIAGNOSIS: CAD with native single vessel disease, LIMA to LAD and LIMA to RIMA Y graft-patent, LIMA to RIMA Y graft to PDA occluded, impaired LV function.

Please help me.

The report findings you provided state that he underwent a 3 vessel bypass surgery in which one of the grafts is occluded (the segment of the right internal mammary artery to the PDA (which is the branch off either the right coronary artery or the left circumflex).  Impaired LV function suggests that his heart contracts much less forcefully than a normal heart -- and most likely due to his underlying coronary artery disease (i.e. an ischemic cardiomyopathy).

If he is experiencing further symptoms such as chest pain, shortness of breath, or fatigue, it would be of importance that he sees his primary cardiologist for further evaluation.
