Pain Chest area

Had bypass surgery in 2004. Taking medication for BP, Cholesterol and Blood thinning (Cartia). Not a diabetic. Continue to have severe chest pain (sometimes for 1-2 days or more) in cycles (say 2-3 weeks). The pain is between rib cage and the surface above rib cage but just below the collar bone. Not sure what triggers it. Blood tests fine, Treadmill Tests really good. Walks 45 mins 4-5 days a week. Nitro spray does not reduce pain at all. Couple of times had intense pain and after Hospital visits all tests came o'k. Over the counter pain medication reduces it a bit but does not go away. Even did a Treadmill exercise when pain was present and heart is fine. Could be musclar skeleton disorder ?
