Post bypass symptoms

My father had a triple bypass just this last fall.  He tells me recently that he still has a hard time getting a good breathe.  That he has times of being light headed.   He has a doctors appt soon, but I just want to make sure we are asking his doctor all the right questions...

What could be the issue here??

One of the bypasses may have disappeared and not healed in.  The symptoms is shortness of breath when this happens.  If this is so, an angiogram will have to be done and possibly a stent put in to hold open the artery that was there before the bypass.  This happened to my mom and it was her main left artery (most serious one).  She had a balloon done and a few years later had to have a stent put in.  Depending on which artery may be involved will determine how serious it is.  He should have an appointment very soon.
