Pressure feeling near carotid

I had my bicupsid aortic valve replaced two years ago at cleveland clinic. Prior to surgery I had a very low blood pressure and occasional syncope episodes and orthostatic hypotension. Since the surgery, I now have hypertension, lightheadness most of the time despite any positional changes and a pressure over the carotids as if i am wearing a tight turtle neck. Recent ECHO normal, gradient of 11 and mean gradient of 6. CT chest, abdomen and pelvis normal. Endocrinology evaluation negative. ENT and neurology evaluation for lightheadness negative. Any recommendation?
