Plan: Evalute cardiac contractility
This real time ultrasound study demonstrates strong cardiac contractions without evidence of akinesis. The left ventricle diameter measures 5.65 cm in diastole and 4.3 cm in systole. The septal wall measures 0.31 cm in diastole and 0.65 cm in systole. The posterier wall measures 0.62 cm in diastole and 1.13 cm in systole. The valve closure is good without vegetation. Estimated M-mode diastolic volume is 156 ml and systolic volume 109 ml. The estimated ejection fraction is 30%.

Dear Laura
Your degree of LVH would not cause the low ejection fraction. Obesity, smoking and high blood pressure will practically guarantee the development of serious heart problems in the future. You need to stop smoking today, and work on losing a significant amount of weight over the coming months. The low ejection fraction could be a function of your high blood pressure (if it has not been controlled for a period of years) - this will need to be treated aggressively. It is also possible that your weight interfered with the ability to obtain clear echo pictures and that the heart may not be as bad as the test indicated. Depending on your age, coronary artery disease may still be a possibility. Regarding heart failure, you are certainly at risk of developing it.
I hope this has been useful. Feel free to write back with further questions. Good luck.
