: I was standing at work, when my heart was pounding 4 times. I felt a rush
of blood go to my head. It felt like I was lifting something heavy. So I
took a deep breath and relaxed. It felt like I could have passed out. I run
2-3 times a week for 30 minutes with no problem. I have a family history of
heart disease. Could this be a sign of a heart problem?  My triglycerides
are in the 500-600 range.

Dear Mr. Steger
Although very distressing to you what you describe is a very common complaint of a number of people. Although it is not possible for me to make a diagnosis without examining you what you experienced may have been due to extra-beats in the heart, which could arise from anywhere in the top or bottom chamber of the heart. These are usually of no consequence unless there is some associated structural heart abnormality.
I suggest that if these continue to occur frequently, and are causing you to feel faint or even pass out, that you see your doctor at which point he may consider obtaining a 24-48 hour recording of your electrocardiogram to evaluate the frequency and character of  any ectopy. In the meantime I suggest you avoid any medications such as cold medicine and other substances such as nicotine and excess alcohol. There is an excellent review about palpitations in this forum answering a question posted on 1/5/98 and answered by SGM.
If you have any further questions or would like to be seen here at the Cleveland Clinic by one of our  cardiologists specializing in the field please feel free to call  1-800-CARE to set up an appointment.
