Recent RVH diagnosis

Recent RVH diagnosis

I have recently been diagnosed by my cardiologist with RVH and have been referred to a pulmonologist for further evaluation.  Naturally I am concerned after reading what limited resources I have been able to locate.  I was diagnosed with left CHF about six years ago but it is well managed as I have a very competent cardiologist.  The recent diagnosis of RVH is a result of a checkup for breathlessness, palpitations, aching (not sharp pain) in my chest with discomfort in my arms with activity, abdominal distention, significant peripheral edema and related sudden weight gain, feeling tired, and restlessness.  This has developed since my last full workup and cath about four years ago although I have regular visits to my cardiologist. She performed a physical exam (peripheral, neck, heart sounds, lung sounds, etc.), an EKG, echo, and full panel to arrive at the diagnosis and referral.  Additional furosemide (currently 100mg daily in three doses) allowed me to lose 12 pounds of fluid over a week's time. My question:  Where can I find reasonable, honest information and given my circumstances what can I reasonably expect as an outcome?  I consider myself to be a stable, sensible person and would simply like to have some expectation going forward.  Thank you.

It's hard to say what the cause of the right sided heart failure is at this point, without seeing all the data, but if I were to bet on it I would say that's caused by your left sided heart failure which happened to have progressed over the last couple of years.  You will need more aggressive medical therapy and this will likely improve the right sided symptoms.  Biventricular dysfucntion, as is the case in your situation carries much worse prognosis than left ventricular dysfunction.  However, if you do respond to therapy and your heart failure stabilized again, you could go on for a while, just as you did the first time around.  It is very difficult to quote prognosis with heart failure because no two patients are the same and we have certainly been surprised many times in the past.
