Right Heart Catherization

Several Echocardiograms since late 2006 have indicated possible pulmonary hypertension (due to COPD). I understand there can be no Dx or treatment of PH w/o first having a right heart cath. I can think of more fun things to do. *sigh*

I've read of the RHC being done via the groin and via the neck. I also once heard thru the wrist as well. WHO determines where the RHC is started? Does the patient have any say in where it is started? What would be the reasoning for each placement of the start of the RHC?

Where can I find a good, detailed, graphic description of the procedure?
