Risk of 2nd CABG surgery

Risk of 2nd CABG surgery

Thanks for taking my question. I am a 62yr old male diabetic with hypertension and hperlipidemia and have just been given a risk of 3-5% for complications by CCF cardiologists to undergo a 2nd bypass. This is mainly to replace a thrombosed and apparently nonfunctional LIMA following a LAD stent implant with severe LIMA spasm. The RIMA is the proposed replacement. My last stress test showed a large area of ischemia both in the anterior and inferior areas served by the LAD. My 1st triple CABG 5 yrs ago involved simultaneously both AVR and aortic root repair and was quite a traumatic experience that took months to recover from and left me with arrythmias. Other Cardios are against taking the risk. Despite a full drug regimen including Nitropatch, Cardizem,Diovan,Plavix, Asp,Coumadin and Glucotrol I still get nausea and chest discomfort on even mild activity. I have a few weeks to make a decision. I would like to get your comments and those from anyone else in a similar predicament. As always I highly appreciate this opportunity to express my problem.

chirs,

thanks for the post.

Sorry to hear everything you've gone through.

There is a large section of your heart not receving full blood supply. This would cause me significant concern.

You do have to weigh the risk and benefits, but generally someone with significant ischemia I usually propose revascularization. Unfortunately I cant comment too much on your case without knowing your coronary anatomy.

I woudl discuss your concerns with your CCF cardiolgist and maybe he/she can discuss the case with your local cardiologist who is expressing reservations.

good luck
