Solution for three vessel disease..

LAD: type 3,Proximal,mid and distal LAD diffusely diseased with 90% stenosis
Diagonals:D1 and D2 has ostial 80% stenosis
Intermediat:NIL
LCX : Proximal LCx has tubular 60% stenosis.Distal LCx has luminal irregularities to a maximum of 40%
Marginals: OM1 and OM2 has diffuse disease to a maximum of 90%
RCA:RCA has ostial 80% stenosis
PDA/PLV: PDA has a tubular 70% stenosis
LV: not done
creatinine : 1.8



what is the right solution sir?

had angioplasty on aug 12..
Now he is feeling ok.

Are you saying that you had a procedure done 1.5 months ago? and something has gone wrong? or are you still at the beginning with no procedure being done yet?

Cardiologist suggested us it must be done as soon as possible and we had no other options except to do angioplasty.
It is about One and half months now after doing this.And we are in such a worry to hear now that it will give no permanent result.
We dont know what to do next.Help us please

Look at what the report says about the left anterior descending vessel alone. It says it has disease spread down the top of the vessel, through the middle and the bottom. There is absolutely no clean area in the vessel and stenting would be a nightmare. Perhaps the Cardiologist feels putting a stent at the top will suffice or something? but in reality the whole vessel is diseased. I have my complete left artery stented from top to bottom with 5 of the longest stents available. After just a few weeks, three of those blocked up. Then there are the diagonal vessel branches off the LAD, which are also diseased. The Left circumflex is also diseased as are the Obtuse Marginal branches off of this. Then there's the Right artery with an 80% blockage. Then there's the posterior descending artery with a long blockage. I really can't see stenting being successful at all. If I could go back in time, to 7 years ago, I would approach my disease very differently. I would have just kept to medication, diet change and see how well nature dealt with my heart first. I felt better before all the stents and bypass and all the interventions just made things worse time and time again. I think I had about one year symptom free out of 7. Nature can do some remarkable things and has built in measures for many people. Apparenly I had a long total  blockage in my left artery for years and yet I knew nothing about it. This is because nature opened up some natural bypass vessels to deal with it, giving a good blood flow. Decisions are difficult and you need a cardiologist who is honest with you. The one who suggested stenting sounds quite extraordinary to me. If we were talking about disease in one vessel and just one or two blockages, then yes, there is a high probability stenting will help. If you do decide to go for stenting, then I ask you google catheter FFR and read up on it. Ensure the cardiologist uses this technology to check every artery before and after stenting to ensure he has done the best job possible.

Angioplasty would not produce the results needed with such extensive disease. The disease is diffuse which means it runs all along the arteries. This would basically take around 20 stents and nobody would insert that many, it's far too risky. It is also likely many stents would re-block in a short period of time.

There are only 2 options available in such as case. 1) control as much as possible and as long as possible with medication. 2) bypass. If there are no problems with symptoms now, then I would choose option 1 but make severe lifestyle changes. Healthy diet is essential. Perhaps a nuclear scan would be a good indication of just how things are doing with the heart. This will reveal if any natural bypasses (collaterals) are helping.

Was in hospital for two weeks because of severe chest pain and breathing problems.Now he's at home.No such problems now.
