Tricuspid leakage

I have the following symptoms:  Shortness of breath; Unable to walk more than 20 feet without my legs hurting and getting short of breath. My height is 5'5 and I weigh 200# so I know I'm overweight for my height. The problem is that I have alot of lower extremity edema. My feet, ankles and legs stay swollen 24/7 even taking a fluid pill. They average about 2-3+ edema all the time. I had an echo 2 weeks ago and it showed the tricuspid valve has minimal leakage. I know the left side of the heart indicates CHF. I don't have CHF (I guess). I do have RA and it's still unclear if I have Lupus. I went to a second opinion and the RA was confirmed. I'm on Methotrexate, Plaquenil, ....But all this swelling happened years before this. I had s/s of MI and when I went and had an EKG it showed I had a Septal Infarct. I am a Registered Nurse. I work everyday and I know alot of times, male physicians minimize women's complaints. All of my symptoms are objective and visual but I feel I'm not getting ANY help!!!

It may be caused by other organ systems such as liver or kidney problems, such as nephrotic syndrome. It may also be caused by multiple blood clots in the lower extremities. I cannot tell these apart from your description, but a normal echo with a normal functioning heart does not fully exclude heart failure, as there is such a thing as diastolic dysfunction which occurs in patients with normal ejection fractions on echocardiograms.
