Uncontroled BP

I'm 70 yr old with a history of HB for past 30 years
have yet to find any medication that has lowers my
BP and keeps it down. I can go for weeks with BP
145/42 to 218/110 most of the time my systolic is 145 to
187 or 90 plus my diastolic is always very low in the low
50 to 62 or 65. This up and down makes me tired all the
time. I have kidney failure but under control.
It's my BP that has put me in the hospital so much. Last
time was this last weekend with BP 187/86 EKG normal
blood gas normal pain was 10 felt more like 12 am now
home feel okay but can feel the chest pain building up
again. I take care of my 15 month old great grandson have
been since he was born. Had a stroke the day before he
was born in hospital and therapy a total of 6 weeks.
Why would the systolic and diastolic be so far apart?
