Urgent help - ejection fraction = 30% and LA and LV size are increased.
My Father 62 year old 220 LB has following report after suffering from breathing problem last night. He is in the hospital. They are removing excessive water from his body. We are thinking moving him to a better hospital. Can we implant ICD immediately? Please suggest next steps:

2D ECHOCARDIOGRAPHIC OBSERVATIONS:
LA and LV size are increased.
No LV hypertrophy.
Generalized LV hypokinesia
Overall LV ejection fraction = 30%
RA, RV size and functions are normal
Aortic Valve Sclerosis and Mitral annular calcification
Rest all cardiac valves appear structurally normal
No clot, no vegetation, no pericardial effusion
IAS/IVS are intact
No shunt across great vessels
Subcostal View = Dilated IVC
DOPPLER STUDY:
Mitral Valve Flow=E: A ratio is s/o Diastolic Dysfunction
No mitral regurgitation
Aortic valve flow: No aortic regurgitation
LVOT gradient not significant
Tricuspid valve flow: No tricuspid regurgitation
Pulmonary valve: no pulmonary regurgitation
Conclusion:
Dilated left heart and reduced LV function.
Generalized LV hypokinesia
Diastolic Dysfunction
