In April I went to the ER due to severe chest pain radiating up my neck, shoulders and back.  I had been previously diagnosed with pleurisy for approximately 2 years and given numerous rounds of Levaquin and Decadron. Close to 18 rounds of antibiotics. I would go to the local ER and they would see a high white cell count, mild grade fever and automativcally assume I had a bacterial infection that was causing pleurisy.  Over the two year time frame my bouts became closer and closer together.  Antibiotics did nothing however, the decadron shot would relieve the symptoms in hours but would recur sometimes 2 months and towards the end of two years my relief lasted for a few days. My primary care doctor in March conducted an EKG and sent me straight to the ER cause he thought I was having a heart attack.  I arrived at the ER they also did an EKG and stated that it was fine and that my primary doctors machine read wrong.  Again diagnosed me with pleuritic pain, gave me and antibiotic and a decadron shot.  Felt good for 2 days.  Symptoms came back. Also, during this time I was seeing a pulmonologist because they were stating that it was allergies that was causing the recurring pleurisy.After this last time, I had an appointment with the pulmonologist, presented with a heart rate of 162.They gave me another inhaler.  A day later I saw my primary physician again.  He proceeded to give me another antibiotic and told me again he believes it's allergies. The next day I went back to my hometown in Kingsport, TN and went to an ER there. Within 2 hours of examining me, they concluded that I was experiencing a pericardial tampanode. They ran numerous tests, including ecocardiogram and transported me to Johnson City Medical Center.During this time, my blood pressure was around 90 over 70 and experiencing tachycardia.  They immediately did surgery.  The vascular surgeon conducted an open heart incision over my sternum and put in place a pericardial window.  They removed 400cc's of fluid and inserted chest tubes.  I remained in the hospital for 8 days. All but two in ICU. During this time I had numerous doctors running all kinds of tests.  Out of all the tests the only thing that came back positive was for the Coxsackie Virus. I am 30 years old, 29 at the time but work with children. The rheumatologist, immunologist and cardiologist feel that this may have not been the cause of the pericarditis.  I was discharged from the hospital on Colchicine and Sulfcrafate. Two days later I was back in the hospital for the same pain.  Fluid had regathered around the heart but was draining slowly.  Oxygen levels were 58. Blood ox confirmed this level.  I was readmitted to ICU for about a week.  More tests ran.  CT scan indicated a slightly enlarged liver and spleen, fluid in chest cavity and pericardium.  Infectious disease wanted to take a liver biopsy but the vascular surgeon disagreed.  They also wanted to conduct a bone marrow tap which most doctors were against.  Neither were done. They changed my anti-inflammatory to Relafen and I was discharged again.  During that stay my white cell count would jump to 29,000 and then in 10 hours drop to 9,000 and I would run a mild fever and sweat a lot. It did this for days. Numerous bacterial cultures were done. Nothing ever showed up. HIV test completed.  It was negative.  I went home for 2 days and the symptoms came back. Chest pain radiating through both shoulders, back and surrounding the rib cage.  They felt it was Post Cardiac Injury Syndrome.  They decided to put me on steroids.  I started at 120mg and 6 days later at discharge I was on 60mg.  Since then I have had a relapse as they decreased the Prednisone. The first was at 2.5 mg and SED rate was very high, C Protein factor was also high as well as a high WBC count. They increased the Prednisone back to 40mg and the pain was gone in about 10 hours. I have been gradually coming off of the prednisone. I was on 5mg for about 5 weeks when the symptoms started comiing back.  It has been a gradual onset. Starting with severe pain in my left ribcage, to the point that my ribs felt bruised all the way around to the floating rib, then would gradually radiate up my side into my left shoulder.  The left shoulder pain has decreased but now the pain is in the center of my chest, under my left breast and in the middle of my back. The pain radiates up my neck and into my ears. I am on 4mg of prednisone. My primary care physician prescribed me a narcotic for pain and it does not ease the pain.  It's worse when I lay down, walking short distances and even sitting and my heart rate increases. The pain is gradually getting worse.  I have a CT scan scheduled in a couple days and am trying to wait it out. By the way the Relafen caused swelling in my joints and arthritis like symptoms so I was put on Naproxen 500mg twice daily. The meds do not seem to be working if it is inflammation.  I feel that the Prednisone is masking whatever it is that is going on, but not for sure. I'm completely clueless. Does anyone have any ideas? I have dealt with this for a very long time and I thought it would be over once they found the pericardial tampanode and did surgery.  Also, will the pericardial window ever heal? Any ideas would be much appreciated. Thanks in advance.
