ablation with mitral valve regurgitation

If you have afib and cardio versions and drugs have not worked, would it be better to repair the mitral valve regurgitation before a ablation? Seem to me even if you do the ablation, with the MR, chances are the proceedure will not work, or last?

Repairing the mitral valve is major surgery... so undergoing valve replacement because of afib is not justified IMO. However, if you already have severe mitral valve regurgitation AND afib, then surgery is usually justfied.  In that case, it seems to me that a MAZE procedure and removal of the left atrial appendage at the same time as the valve replacement is the way to go. An ablation at that point probably wouldn't work because of atrial enlaregement.
