heart sac

My husband had bypass surgery 17 years ago and has had several other things done since ,such as balloons and stents. He is having alot of problems recently and seen a heart surgeon and was told he needs one of his grafts replaced since he only has two left open but, after a CT scan showed that his heart has fused to his sterum and rib cage it would be almost impossible to do this. He said when they did his by-passes they didn't attach the sac back over his heart , and therefore this is the result. My question is: Why was this not done and is this a common thing that happens during heart surgery.
