hi, my wife had a stress test that showed a very large anterior apical and inferior infarct w/ no evidence of ischemia. the gated wall motion study showing a dilated left ventricle, global depression of left ventricular function, ejection fraction of 38%. what does this mean?  thank you

It means she had a large wall infarction, and her heart function is about half what it should be. This does not mean that she has congestive heart failure, but that she is at risk for this as well as sudden death. I do not feel that the test can absolutely rule out areas of ischemia so I would order a cardiac MRI or PET scan and if there is any viability then I would perform a diagnostic heart catheterization and possible intervention or bypass depending on the findings.
