hypokinetic interventricular septum

hypokinetic interventricular septum

Dear Dr. I am taking care of my 76 year old mother-in-law who has a history of a heart attack , open heart surgery, placement of a pacemaker and parkinsens disease. We had an appointment today with her neurologist and she gave me results of her echocardiogram and said I should take my mother-in-law to her cardiologist because there is evidence of "very hypokinetic interventricular septum" and "subtle almost akinetic interventricular septum with anterolateral wall compensatingand less so the anterior wall for the dysfunction highly suspicious for ischemic cardiomyopathy. We can't get an appointment for a while, could you please explain this and should we do something right now? Thank you Mindy

With a previous history of a large heart attack and depending on what the infarct related coronary vessel was, one would certainly expect that there would be wall motion abnormalities as described on the echo.  I am assuming that she had an echo done around the time of the heart attack.  If so, than comparing the previous echo to the one done recently would be of help in trying to decide whether these changes are new, thereby, necessitating further workup.  as long as she is not having worsenig chest pain symptoms, lightheadedness, or worsening shortness of breath then nothing needs to be done at this point.  Just follow up with the cardiologist.
