interpetation of results of medical test

i am 54 yrs. 5ft 8in 145 lbs. Marathoner running average  8 to 15 miles a day for over 30 yrs. Now only running 3 to 5 miles a day., am fit with no health problems in my life except hernia operation and take no medications. I had an Echocardiography results:Left ventricle:concentric LV hypertrophy, uniform brisk contractility, good systolic fuctionLVEF 77% Left atrium: normal RA andRV normal IVS and IAS  normal  Mitral Valve:normal, mild AML prolapse, normal diastolic gradients, mild mitral regurgation  Tricuspid Valve: normal PA Pressure normal (15mm HG) Aortic Valve: Bicuspid, calcified with mildly restricted excursions. Moderate aortic stenosis, peak gradient 41mm HG Mean 21 mm HG mild aortic Reguritation  Pulmonary Valve: normal  I would like to know what this shows and  possible suggestions as to health risks in the future. Thanks  steve

The LV hypertrophy probably isn't related to the moderate aortic stenosis due to bicusp aortic valve, unless the stenosis is underestimated.  I don't think that's the case because of how active you are.  I think the hypertrophy is related to the fact that you are quite an athlete.  The mitral valve isn't very concerning but needs to be followed by a cardiologist in regular intervals, every one to two years or so.  The biscup AV with AS and mild AI is concerning and needs further regular rearssessment in the future.  You should also have your proximal aorta screened with a CT scan to make sure that there aren't any aneurysmal changes there.  At this point there isn't any need to do surgery, but chances are that you will need the valve repalced in the future.  It's difficult to say when.  The next echo which will be done in a year or so will show us how the disease process is progressing.  Be very conscious of your exercise capacity.  When that starts to go down (worsening shortness of breath or chest pain with exertion), that will probably be the time to think about the valve replacement.
