is this acid reflux?

Sometimes when i swallow food in large amount its hurt to swallow. When i drink water to flush it down its kinda hurt. Late at night whne ever im doing sometthing then my hurt suddenly hurting like stap pain, and when i breath sometimes i mean take a deep breath my chest is hurting. When i yawn too it hurting. what is this is this acid reflux? or an heart disease, im 24 and im worried. theres a time too that when i lay down then i feel like im gonna have a heart attack. Im scared i dont know what is it. BUt now its kinda lay lowing not like befre though its really hurting, but i still feel it most of the time still the same pain. im really worried.
