left shoulder pain

Periodically I experience left shoulder pain that feels like it has nothing to do with my shoulder.  I havent hurt my shoulder in any way.  It never happens to my right shoulder.  I also experience pain in my chest that most time accompanies the shoulder pain.  It is always in the same spot (in the top of the front of the shoulder).  I am a 49 yr old woman.  I had rhuematic fever as a child, and was told that my heart was slightly damaged at age 11 or so.  I have had an EKG which showed nothing irregular.  When I am experiencing this pain, it worsens if I try laying down.  Do you think this is heart related?
