Where can I get a procedural description of the "maze procedure" used for atrial fibrillation?

The maze procedure is used to treat atrial fibrillation, to decrease the number of epsiodes and to hopefully "cure" it. The procedure involves making a number of incisions in the heart during open heart surgery. More recently, a minimally invasive surgical approach has been introduced with good results. A catheter-based appraoch has also been evaluated, thought the results have not been as good yet.
