missing valve flap

i have a congenital defect which is a missing heart flap.  i have chf, cardiomyopathy, tachycardia and ventricular arrythmia.  i have an icd.  they are still trying to fine tune my meds and my defib/pacemaker.  my question is, what heart rate are we shooting for?  i know that i get very tired and am often really short of breath leading me to believe that too much blood is trying to get thru my heart and my heart can't handle that.  am i even close in understanding that?  I am thinking that they need to slow my heart down to properly function and i am interested in what that heart rate would be.  thanks

The heart rate that you need depends on the congenital defect that you have, as some improve with heart rate and some worsen. I suspect that you have a PFO or even an atrial septal defect, which cases a left to right flow and right ventricular dysfunction, manifested by lower extremity edema, shortness of breath and ascites. If this si the case, and except in severe advanced cases, the treatment is to close the defect. The heart rate you should be striving for is whatever your body decides you need for that situation ( you state you have an ICD not a pacemaker), so I would place you on some beta blocker for arrhythmia suppression and CHF prevention but not enough to cause you to be hypotensive, or dizzy. Most of the medication you are on are meant to help your heart to work less, and increase its longevity, and to diminish arrhtyhmias or ICD shocks.
