Dear Dr.,
First let me say I am a little embarrassed about posting questions as often as I have and I hope you and the people who use this forum will forgive me.

Early last year I was diagnosed with mild mvp w/mild mr (and mild tr, IST, NCS). At that time I didn't have a murmur. At my recent visit to my EP he heard a murmur and has now prescribed antibiotic prophylaxis. My questions are: 1. Is this possibly from me increasing my sodium and fluid intake (for the NCS), thereby increasing my blood volume? 2. If so, does that mean it is ok? 3. What are the other possibilities? 4. Is this an indication that my valve may progressively get worse?

Lots of different factors can cause a murmur to worsen. If the valve itself has deteriorated, that can cause a worse murmur. Differences in heart rate and blood volume can also accentuate a murmur. Different positions of the body (sitting versus standing) can also cause a murmur to sound different. Different examiners may also hear murmurs differently. Probably no need to change your fluid intake based on the murmur, but may not be a bad idea to have an echo if one has not been done in a few months.

What do you do when you find out that you don't have a heart murmur, that the person relaying the dr.'s message just took a guess?!!!!!  Can you imagine?! Nothing like getting the message straight from the horse's mouth and that was nothing like it!!!!!

Does anybody know if you need to take antibiotic prophylaxis for a punch biopsy? I will only need a couple of stitches, but I was wondering if I should mention to the dermatolgist about needing antibiotic proph.  What I could make out from the AHA recommendations was that I wouldn't, but I found an old post, only one, in the archive where a dr. recommended it.
