Have you had a perfusion stress test?  Its a big step to go from a single occluded vessel to open heart surgery.  Is there an opinion that the stent opened the offensive vessel, but that vessel is still causing angina (chest pain)? Medication does not relieve chest pains?

There are vessel spasms that require a special test to diagnose and that can cause chest pain.  I would get a second opinion before an open heart surgery for a single by-pass.  I have a 73% and a 100%  blocked arteries for the past 6 years and medication serves my condition very well.  Everyone is different physiologically so one can't identify with another similar situated individual.

Sometimes to by-pass a blockage that has developed a natural by-pass can be probablematic by decreasing the blood flow with the natural by-pass.

Are you having a bypass or angioplasty (Stenting)?
Are you saying your blocked artery grew Collaterals but you still feel angina? If so, this is not uncommon. If you feel angina, then your heart is under stress and surgery in my opinion is a good move to improve things. I assume at this stage it has been proved medication has little effect on your angina.
