new onset bigeminy

I have minor congenital heart defects (small VSD, small PDA, mild MVP).  At 47 years old I am mainly fine.  Recently developed Bigeminy (with low BP and pulse, sometimes feeling the palpitations)and seemed to be staying in the rhythm for more than 2 months now.  I understand that PVCs are generally benign, however, with the minor structural defects do I need to be concerned?

Hello,

It is worth having your rhythm evaluated with a holter monitor.  That should help determine how frequent it is.  If it is frequent enough to capture on a 12 lead EKG, that is usually enough information to localize it in your heart within a few centimeters.  This can be helpful if:

1. your PVCs are very symptomatic
2. They cannot be suppressed with medications
3. The decision is made to try an ablation

It is also important to make sure your heart function is stable.  Hopefully you have a good adult congenital heart disease specialist.  It may be worth seeing an electrophysiologist as well, especially if the bigeminy is constant.
