non union sternum

my fiancee had a bypass surgery in July 2010 , due to being a smoker for 35 years he coughed so hard that the wires pulled through, detached parts of the sternum and has a non union sternum, 1 inch appart at the lower part of the sternum, without cleaning off scartissue 3 titanium plates where used to fix the problem in Dec 2010, 2 days after that surgery it came to stress fractures of the at least 3 ribs, that are still moving and now at least 5 of the screws holding the titanium plates came loose and moving "in and out" . the extreme pain is unbelievable and no doctor seems to know what to do next and they are happy whenever we leave and they can send us somewhere else. they want to take the plates out but nobody can tell us how instable he will be after that,
we would like to be able to send the CT scan and reports to some clinic, surgery or ortho to discuss this case with a Dr. before they start cutting

What you are describing sounds like a very complex surgical case that would best be addressed by a center with experienced physicians/surgeons.  This can usually be found at a tertiary referral center or any academic/university-based health system. I would suggest seeking a referral from your regular physician. If unsuccessful, I would try using the internet to locate the closest health system where you can seek a 2nd opinion for further evaluation and/or treatment strategies.
