palpitations and chest pains

hi, i am a 28yr old female w two children. 5yrs and 1 1\2. this all started w my second pregnancy and reared its ugly head approx 3 mnths after she was born. i had my first hard palp in my 9th mnth of pregnancy, it was a HARD thump in the chest that scared me. i hv since had numerous holters, ecg, echos, stress test w &w\out thallium, a eps study,cardiac mri, and ct of the heart w contrast. all were considered normal, except t waves are a little flat(unspecific abnormal t waves-my twin has the same thing), one echo said mvp w regurg then second and third said no. holters showed in one day 65pvcs(and i DO feel them and they scare me) and one time i was put in the hospital for a 5 beat irreg ryhtm that they are clueless as to y it happened. i am on atenolol 1\2 of 25mg daily for pvc and for fast hrt rate upon exertion. last night i felt a horrible flutter around my chest of my heart beat when lying down and scared me! it lasted approx 15 secs. what could that have been? & y are these things happening to me if they can not find anything wrong w my heart? i also get ALOT of chst pains! i hv been diag. w h-pylori & reflux & strted the prev pac w\out takin biaxcin yest. i was diag. post partum thyroiditis 3mnts after the baby & was put on syntroid and propanolol. worst tsh was a 91 now it flucuates between 5-10. only on synthroid 12.5 1xweekly.
1.what could be happening, and does any of this sound threating?
2.what couls the flutter have been?
3.y so many pvc's if they can't find anything?
4eps study normal so y the 5beat rythym thing>

Hi Kacey,

I can understand why you are having such a hard time.  It is difficult having symptoms that cannot be improved compounded with no understanding why they happen in the first place.  For most cases, we do not understand why some people have PVCs and other do not, and why some people that 15000 per day don't notice them and some with 5 do.  We just don't know how to answer these questions with concrete facts.  We do know that people with normal heart function, normal EKGs, and your in case, normal MRI, stress test, EP study and CT -- have a very good prognosis in terms of sudden death.  The problem is that PVCs make some people feel so poorly that we cannot convince them that they are not going to die and this fear ruins their life.  From the test results you describe, this does not sound life threatening.


2.what couls the flutter have been?

There is no way to know for sure without having a monitor to record them -- which takes a little bit of luck.

They could have been PVCs, PACs, combination of PVCs and PACs, plus any number of other conditions.  Flutters are a fairly common concern that people ask about, but we need a monitor to have an idea of what it is.

3.y so many pvc's if they can't find anything?

There is not a good answer for most people that have normal hearts and PVCs.

4eps study normal so y the 5beat rythym thing?

EP studies do not give that kind of information.  The are not good for diagnosing or determining the etiology of PVCs.  I would do an EP study for PVCs if I was considering doing an ablation for them and I would only do a PVC ablation as a last resort after medical therapy.  I would start with a beta blocker and only move to a sodium channel blocker if your symptoms were very poorly tolerated and you fully understood the risks/benefits.

5.can hypo thyroid cause irregular rythms?

The goal would be to normal the values for TSH and T4.  If you still have them with normal values, it is a stretch to say that they are the cause.  Could they be related??  If you developed an autoimmune syndrome that effected your thyroid, it is possible that this is also effecting your heart.  This is also a stretch.

Hello,

A few months ago i went to a doctor because i had a lot of pvc's.  After a lot of tests he told me that a have a normal heart and that my pvc's are benign.  But he also told me that ten pvc's in a row could be dangerous.  Why is that?

I'm also concerned about my brother.  He is having pvc's in a row (7-8) at a low heart rate (50-60bpm).  What is this?

ten pvc's in a row like that is dangerous i was told b\c it then turns into v-tach.  i've been dealin w pvc's since my 2nd daughter was born and they do not kno why. i had 65 pvc's in one day w a 24hr holter monitor on and i felt at least half of them. and the cardiologist says that is ok. STRANGE!! he says my body is just sensitive and i feel them more than others.  its confusing how one test will show only one a day and then to have 65 another day. there has to be a connection somewhere....
