rheumatic fever

My 13 yo son was diagnosed with rheumatic fever about 3 yrs ago and has been on 250 mg of pcn 2 x per day since then. He had pericarditis and involvement of the mitral valve at that time, but it has since resolved completely. Recently he has been rebelling and about a week ago, his stepmother found several of his pills hidden in his room (this was not the first time). Last Tues he developed a sore throat, malaise, and fever. His father held him out of school for three days, but did not take him to the doctor. My son stated on the third day that he felt better, but still showed signs of decreased activity on the weekend. I am concerned that he may have gotten strep.
Should I be concerned about this? Is there anything I can do for him at this time? Is there anything I should watch for now?

Antibiotic treatment of strep infections decreased the overall risk of sequellae from the infection.  

Unfortunately Im not a pediatrician. If you have specific concerns about his health,especially with issues of infection, I think you should consult your pediatrician.
