Yes this is a serious situation and requires CABG. You must remember that we have 3 main arteries to feed our hearts, the LAD the LCx and the RCA. All 3 have serious blockages in them. In some people, tiny vessels open up called collaterals which are natural bypasses. However, you need a good supply in at least one vessel to open these up and put them into operation. The supplying vessel has to be within the vicinity of a blockage also. Your RCA has PDA and PLV are being filled via collaterals which will be from either your upper LCx or RCA.
