sinus of valsalva aneurysm

I was diagnosed with Takayasu Arteritis 12 years ago, 3 months after having an aortic valve replacement (homograft) and ascending aortic root replacement.  Several years ago, they found a sinus of valsalva aneurysm.  It has grown 3mm over the past year and last years measurements were 20 x 13mm-bulging into the left atrium.  I seem to be having more and more PVC's (even on atenolol 100mg BID).  The PVC'c are so exhausting and pretty constant.  Could the increase in the number of PVC's be related to/caused by the aneurys and at what size-generally- it surgery considered.  I have been told that it is inoperable.

It is always difficult to say why you are having more PVCs -- they can sometimes be cyclical relating to changes in stress, diet, sleep, etc. and they can also have a structural component.  It is possible the PVCs or PACs are related to your heart condition/aneurysm.  There is no way to know for sure.

The decision of when to operate on a sinus of valsalva aneurysm related to a aortic homograft is not easy and there are no strict numbers that I know of.  It would relate to the circumstances like the size, progression, and appearance on echo/CT/MRI.  I would recommed a subspeciality center known for familiarity with these matters like Cleveland Clinic, Mayo Clinic, The Johns Hopkins Hospital -- certainly not limited to these, but a high volume center is prefable.  This isn't always easy if it means traveling, but experience does count.

If they feel that it is inoperable but they think it needs surgery, please visit a center like the Cleveland Clinic, Mayo, or experienced cardiothorcic surgical center.  I can't tell you how many cases of 'inoperable' coronary and aortic disease I have seen walk out of the hospital after their surgeries.
