ventricular hypertrophy and EKG abnormalities

Hx: mitral valve prolapse w/3+/4+ regurg; small patent foreman ovale; mild pulmonary hypertension; isolated systolic hypertension, diffuse peripheral vascular disease/atherosclerosis; 52 yo non-smoking, non-drinking, technically underweight female, with hx of strokes and TIAs.
Meds: Lozol, Avapro, Dilantin, Cardizem CD, Pletal, Labetalol, Coumadin, Vit.
I recently had a stress echocardiogram because of chest pain and fatigue and don't understand all the results. My cardio doesn't engage in many explanations and he said (via the sec'y) that the results were fine but I'd like a little more knowledge so I am coming to you.
Test Result Diagnosis:
Clin. Susp. of CAD (cath at CCF a few years ago was clean)
Resting ekg=nsr, NSPST CHGS., POOR R WAVE PROGRESSION
@6 min - .5-1mm DOWNSLOPING ST DEPRES. INF. & V6
@9 min - 1 mm Downsloping ST Depres
@9m52s - had to stop due to SOB and faintness w/75% max aphr (hr 126)
@3 min recovery - T WAVE INVER. V4-6 INF. Ret. to Baseline
@ 6-15 min recov - O2
BP raised from 148/42 to 168/00 (0 readable diastolic is not uncommon for me) during stress test (which is really good for me b/c the SBP is usually well over 200)  
Echo Interp:
Inconcl Stress Test due to abn EKG@rest & 126 HR
Abv avg exercise capacity
MILD CONCENTRIC LVH
RESTING LEFT VENTRICULAR HYPERTROPHY
Q1: What do all of the capitalized phrases mean to me? Are they important to know about or they just incidental observations?
Q2: Why do I have "abv avg exercise capacity" when I can never finish the test?  (all prev tests said "below avg capacity)

Thanks for the post.

The bottom line is that your baseline echo is consistent with a history of high blood pressure.  The stress portion of the test is totally inconclusive -- the heart rate achieved was not at least 85%, so the possibility of coronary artery disease has not been well evaluated by this test.

Exercise capacity does not equal high heart rate.  Some people are able to achieve good exercise capacity without a high heart rate, which you may have done.

All of the phrases that you listed are technical phrases that require more than defining for proper understanding.  I think the way I answered your question will serve you better.

By the way, I can think of no circumstance by which I would relay these types of results via my secretary.
