worried about spouse

Should we be concerned about my 68 year old spouse's swollen ankles in the p.m.?

Having swollen ankles is never a normal finding and should always be evaluated by a physician to determine the cause.  It may be the result of something completely benign such as dependent edema or faulty valves, very common if the episodes occur at the end of the day and resolve by the morning  However, it should be worked-up completely to ensure there is not another problem with the heart, liver, or kidney.

Yes.  Swollen ankles are not 'normal,' and can indicate circulatory problems.
