Coumadin and Blood In Urine

Have been on coumadin almost 2 years for mild A fib. Out of the clear blue sky 09 April I had a toilet bowl full of old blood, enough that one couldn't see the bottom of the bowl. Absolutely no other symptoms and no medication changes.

INR at ER was 1.5. Urine culture indicated budding yeast and some bacteria. Xray and CT scan showed no abnormalities in urinary tract . Dx was hematuria. One Diflucan was administered and 7 or 10 days of Keflex (I forget which) and sent home w/instructions to continue current medications. Family doctor referred for cystoscopy which was also negative.

Urine cleared visibly for a few days and then returned, some episdoes darker than others but never enough to cloud visibility of bottom of bowl. Still no other symptoms. Still always old blood (brown not red or pink).

Should I ask about discontinuing coumadin (other than at ER my INR has been running 2.7)?

Why does the visible blood continue to be brown and never pink or red?

Where can it be coming from?

Thanks, Jerry. I guess that is what we need to weigh - blood in the urine or no inhibition of the clotting factors, which one carries the biggest risk in my situation. I'm 70 years old and other bloody discharge has been ruled out via the above tests and pelvic and transvaginal ultrasound.

Since there are no other noticeable problems I'll probably just "ride it out" until I see my cardiologist in August.

I have been on 5 mg of Coumadin/Warfarin for at least 10 years.  My INR is almost always in the target range 2.0 to 3.0.  I never see any blood in urine, or elsewhere.  I think when I "nick" myself with a sharp implement, I bleed more than I used to, but never a problem stopping.

I believe blood in the urine is one of the "no-no" symptoms and should be addressed.  Maybe your doctor would agree to let you stop coumidin (replace with aspirin) for  a few days and see what happens.  It is possible aspirin may case problems by itself.

Of course, a female may discharge blood other than in the urine.  I am a male so my experience is from that perspective.
