1. maximize medical management with beta blockers, ace inhibitors/ angiotensin receptor blockers, aspirin, aldactone if significant heart failure symptoms
2.  no smoking, exercise / weight reduction if appropriate
3. make sure there is not obstructive coronary disease that can be opened ( a narrowed graft)
4. once all the above done, discussion about whether a defibrillator or biventricular defibrillator is indicated.  People with irreversible low heart function can be at risk for dangerous arrhythmias.  Defibrillator can help treat dangerous arrhythmias in carefully selected patients.  If he has conduction disease in his heart (wide complex on his EKG), he might have a symptomatic improvement with a biventricular ICD -- this is a three lead pacemaker that also functions as a defibrillator.

I hope this helps.
