Heart problem, hypotension problem ?

Hi ! My english is not good, i'm a french canadian from Québec.

I'm a 30 years old women. I can say that I am in good health but have malaises since i'm like 15-20 years old. Lightheadness (like bizarre vision), abnormal fatigue, eat intolerance +++. When doing physical efforts, I can feel bad for a few hours after, extreme fatigue, I need to rest absolutely. I eat great, drink a lots of water. Last year I pass a test (MAPA) to mesure my blood pressure for 24 hours. It looks ok in the day (like 110/80) but at night it drops a lot. I didn't really sleep the night I pass the test because it was inconfortable, and it drops at 70-80/50 when I was pretty relaxed. So i said to myself that when I sleep profondly, maybe its dropping more than that ? And sometimes, when I wake up in the middle of the night and have to go see my son or go to the bathroom, if i'm not really good awake when standing, I experience BIG malaise. At firt, i was thinking about the big drop in blood pressure since it is really low. BUT it's not just that. When I stand up (even if I take my time) I feel like i'm weighting like 300 pounds and my breathing is not great. My heart goes really fast (it must start at like 40-50 BPM, my heart rate is pretty slow when sleeping or when i'm calm... then goes to 100 and ++ BPM just to sit in my bed) and when I return in bed after and sit it takes like 5 min. to my heart to re-ajust and I'm feeling that i'm gone to die, my heart is not beating regularly (it's going fast and slower and faster and after a few minutes, it calm down), it's difficult to breath normaly when it's happen. I become nauseus and bizarre in my stomach and intestins. It's like my heart have a lot of difficulties to re-ajust after that and makes me feel like it's not pumping normaly at all. I'm really afraid that I have something wrong with my heart since I have a lots of problems exercising for so many years. I've take the stress test last year too. It takes like 10-15 min. my heart was up to 190 BPM and everthings looks normal but he writes : sinus arrythmia... something like that. The doctor I consult after said it was normal at that rate (190 BPM). Now, I would like to take the holter test too. But I don't have those malaises everynights. I've been a few months without having it.
But for the exercising, it's since I'm a teenager I have problems. I don't smoke, i'm not overweight (135 lbs.) I lost a lot of weight in the past (was at 210 pounds at the age of 12 and lost a lot of weight at 14) and it's mostly after that that I became to experience more problems with exercising, malaises etc. I was back to 200 lbs at 20 and now, I have a stable weight since 4 years. But I remember at the age of 10 doing a marathon at school and have ALOT of difficulties finishing it and recuperate after, I was overweight at this time but at 10, you're not suppose to have difficulties like that no ? I've take many blood tests that was all ok.
And for the big malaises at night, it happen too when I drink alcool. Just changing positions in the bed when sleeping makes me having palpitations and wake me up. I feel that i'm going to die !! I have really hot flashes, nausea. I was thinking that maybe when I drink a little, I become dehydrated more rapidly then another one but I don't understand why it would happen ? So I don't drink anymore or if I want 1-2 glass of wine I take it at like 6-7pm. and not before going to sleep. I feel like i'm 80 years old and feel very very handicaped by this condition. I'm very ofter tired. Do you think I can have a problem with my heart ? Do you think that if I have a problem it would have shown at the effort test or not ? Thank you and very sorry for my english.

Hi,

I think you should really book a time with a cardiologist and get yourself checked out. In the meantime also make an appointment with your regular doc and get some bloodwork done including liver and pancreatic enzymes checked. The palpitations could be SVT as it happens when relaxed and sleeping, changing positions etc...
With my SVT I experienced lots of low blood pressure, especially after a palpitations attack. It seems the BP goes up the moment the palpitations start and then suddenly crashes and goes down really quickly. After my cardioversion for a very long and severe SVT attack I was wiped out for a good 2 weeks until my blood pressure returned to normal and after my ablation a good 2 months passed by until I was feeling better with more energy.
The doctor can prescribe you medication for low bp which you take in the mornings but to be honest if you start feeling dizzy, then put some sea salt/ mineral salt under your tongue, let it dissolve slowly, eat liquorice, drink coffee all these in moderation have the ability to help raise bp.

I hope you get yourself checked out especially with a cardiologist

I don't think there's really something wrong (like life treatening) with my heart. I'm young, I have those malaises since I'm a teenager too and I'm alive ahah ! Zero history of heart problems in the family, everybody's pretty healthy !
I'm an anxious person too and the doctors I consult always put that on anxiety but I know there's something more then just anxiety. In the day, everything is mostly "normal" with my heart. When I stand up quickly (when I was sitting, relax for a long moment), my heart will go fast for a few second, sometimes head pressure a little bit, but it's ok, it's like that for so many years, I learn to live with it ! And I know it's because of my low blood pressure. It's more because of the fatigue, the malaises I have after doing an effort, the heat intolerance too. I feel handicaped by that, I'm young, I'm healthy but don't feel like it at all :( I would like to go to the gym and feel GREAT, see the benefit of exercices instead of just feeling bad after because i'm exausted :( I'm gonna see my doctor the 27 of june and ask for a Holter, for a few days. He told me that for low blood pressure, there's nothing to do about it. He said that there's no medication for that and all the other doctors I've seen in the past said that too.
For STV, I've read on that... My heart never goes to like 150-200 BPM... I think. And when I have palpitations problems, I don't have it for a long times. And the only times I have a fast heartbeat when sleeping (when I'm lying down), it's when I had drink alcool and it wakes me up). It never happened beside that. It happen when I stand up in the night yes, when i'm pretty fatigued, it's like my heart is freaking out and can't re-ajust by himself right away, it takes a few minutes, even when I'm sitting again. And I feel really bad. And like I said, in the day, it's ok, it's "normal" beside when I'm doing efforts, or when sometimes I stand up quickly after being pretty relax. Wish there's nothing wrong with my heart but with the stress test, everything was normal so... But I need to know and do all the tests I can to see.

And thank for your advise. I know for the salt but never see a difference ? And for the coffee, i'm intolerant to all stimulant. Since i'm a pretty hypersensitive person (sometimes I say to myself that maybe my heart responsiveness is because of my big hypersensitivity to.... everything... ?) I'm a pretty emotive person (TO much emotive...) Maybe there's an impact on my intern organs ? I don't know if it's possible.... I don't think so but who knows !

one other thing you can try are compression hose, some start out with knee hi level and a lower compression then go up in length & compression...this helps with bp, blood pooling and has helped some with hypotension with circulation; I didn't find even the higher mmhg ones helpful but others say they work wonders

Everything you mentioned even the stuff about being hypersensitive to coffee points me in the direction of SVT. Drinking a cup of tea with caffeine was usually a start of extra palpitations for me.
