Help interpreting an echo

For the last several years I have had problems with presyncope.  I also with the last year been developing increasing episodes of chest pain and tightness, palpitations, rapid HR, flucuating BP ranging form 90/50 to 160/88.  I have had several ECG's done with different results coming back from it.  It showed normal sinus to IVCD.  In september I was severe pain in my chest that only last a few seconds, put the episodes of palpitations and presyncope was increasing more frequently.  I was order to do a Holter monitor (which i have no reults from as of yet) and a Echo.  I received my echo today and hoping that if another MD is reading this can interprete the findings for me.  And maybe explain if its anything to worry about.
(I am only showing results that did not come up as an N for normal)

Septum Motion= ?late contractility (RBBB)

ejection fraction = teicholz 59%


I am just wondering usually RBBB present no symptons, howevers with the prescynope and palpitatiosn etc that been having i am trying to figure out if i should be more cautious, or worried then usual, and if a possible solution to this is pacemakers, or medications or if no treatment is needed.

I do not know what all results mean, I had to ask my doc several times for help. I am glad you are getting tested, it is always good to make sure all is fine with our heart, espeically if a person is having some of the problems you posted. How does the doc feel about the echo in general?

LV Posterior Wall, Diastole: 9mm

Right Ventricle Diameter: 26mm
LV Outflow Tract Diameter: 21mm

CALCULATED VALUES:
LV Fractional Shortening: 37%
IVS Thickening Fraction: 24% (The lab says normals are 30-65%)
LVPW Thickening Fraction: 46%
LV Ejection Fraction: 75% (Their lab says normal is between 48 and 72%)

CARDIAC STRUCTURES:
Aortic Root: Small Size
Left Atrium: Normal
Right Atrium: Normal
Left Ventricle: Small size
Pericardium: No pericardial effusion

REGIONAL LEFT VENTRICULAR WALL MOTION:
Anteroseptal Wall: Apical, Mid, and Basal, all Normal
Anterosuporior Wall: Apical, Mid, and Basal: all normal
Posterial Wall: Apical, Mid and Basal: all normal
Inferior Wall:  Apical, Mid and Basal: all normal

VALVE MOTION AND MORPHOLOGY
Aortic Valve: Native valve. Valve area index 1.69cm2/M2 by L: Continually
Mitral Valve: Native valve
Tricuspid Valve: Native Valve
Pulmonic Valve: Native valve

DOPPLER FLOWS
Aortic: Peak velocity is 1.39m/s No aortic stenosis or regurgitation
Mitral: No mitral stenosis or regurgitation
Tricuspid: No tricuspid stenosis. Physiologic tricuspid regurgitation
Pulmonic: No pulmonic stenosis or regurgitation
PA Pressure: Normal Pulmonary Artery Pressure: 26-31 mmHg  Systemic BP 140/80 (Note: I was very scared and nervous during the entire test)

COMMENTS: Sinus rhythm with rate of 40bpm

The doc said there was no narrowing anywhere or thickening, and to not worry abou the small left ventricle, that it was probably always that way, and not a thing to worry about, she also said the same thing about the aortic root, that it went hand in hand with the small left ventricle. She said she was really pleased and to relax.

I noticed your labs has different values from my lab. I know our lab is rated tops in the Northwest, but, I also know that all labs usually have some different values for normal I wish things could be more uniform.

Anyway, hope this helps. I get confused with numbers and such, bottom line I feel is how the doctor feels about the test, and then take it from there. Keep us posted.
