I'm goona pull a guess out of you-know-where but it sounds like anxiety/panic.
I would suggest a combo of a beta blocker and a benzo in the short term to get her calmed down. THis could be Xanax in the short term + inderal (I would say 20mg t.i.d.). If this is effective I would switch her to clonopin for a more long term approach until she can get the PAD under control.
First things first, I would go after the sleep because without it the PAD is just going to keep getting worse and worse.
