Strong pounding heart w/ normal pace

Hi... I am a 52 yr old male, over weight with a history of controlled afib (Rythmol SR 325 twice/day) and untreated sleep apnea.

Over the last couple years I've developed a sudden pounding heart beat that seems extreme to me however the rate is normal. Often the pounding feels like it is slamming at the base of my skull, very loud in my ears and a physically thumping in my chest. It occurs 2-5 days a week. It most often happens at night within a hour or two of going to bed. It often wakes me up and/or will keep me from falling asleep. It can last 15 minutes to an hour and sometimes I have to get up or watch TV to get my mind off it. Often it will coincide with waking from a an intense dream about being angry or doing something physically demanding. However, it can occur in the morning an hour or so after waking.

I am under the care of cardiologist with annual check ups. Other than the afib I've been told my heart is quite healthy. I've complained of the pounding heart beat a few times, have worn a holter monitor with no recorded concerns.

This is beginning to concern me and is a bit scary. I've been told that I may just be extra aware of my heart beat. Any explanation or advise will be appreciated

Let me give you post a bump to see if anything comes in.

I first thought maybe some blockage in the carotid artery would cause one to notice the heart pounding, i.e., working harder to get the blood through the artery.  I could NOT find any connection in my quick look for blocked carotid artery symptoms.

What is your blood pressure when you hear/feel the pounding?  If it is higher than normal, that would be a strong indication it isn't your imagination, I'd guess.

Could it be a PVC?

I've not checked BP during an episode. I should get a cuff. As far as PVCs.. tests in the past have shown some PVCs but cardiologist said they were not a problem.

Jerry, you mentioned giving my post a bump. I am new here so not sure what to expect. Is it common for physicians to respond here? Is that what the bump was regarding?

Just recalled that I had a ultrasound of the carotids a few years ago when the afib was first diagnosed. They said I had the carotids of a teenager and that it was obvious I never smoked which I thought was interesting.

Of course I imagine things can change in a few years.
