When reading an EKG, there should be a gradual increase in the amplitude of the R wave between leads V1 and V4. This is known as R wave progression.
Poor R wave progression is a nonspecific finding. It can be caused by conduction defects , anterior myocardial infarction, cardiomyopathy, pathological conditions and lead misplacemement. This finding on an EKG can only be suggestive, further evaluation should be done to determine the cause.
