Very painful white bump on top of external hemorrhoid

I am in an unbelievable amount of pain due to my hemorrhoids. I am a 15 year old girl who has never really had much problem with her health in general. However, due to my very poor diet/lifestyle habits, I get hemorrhoids. This is not my first time, i have had external hemorrhoids before (i think abt twice) but they never really hurt that bad and so i never researched or went to the doctors. This time, it got REAL bad. Seriously. At first, I experienced slight constipation for maybe 2 weeks. Nothing serious yet. Then on the third week, i flew overseas. While overseas, sh*t got real(literally). It became excruciating for me to poop. Really really painful. I cant even look at the toilet bowl without a sense of great fear. But strangely enough, unlike many people, the great intense pain could only be felt while pooping. Afterwards, i hardly felt anything at all and there wasnt much blood. (Why is that?) I went to the doctors, realised it was external hemorrhoids, got some ointment and stool softening meds. Totally changed my diet, drank loads of water, good habits etc but it was still bad. Then i started to realise that all this while the pain was all concentrated on one spot. I went to the doctors again and apparently, i have this white lump (0.5-1cm) on my external hemorrhoid(2-3 cm) that was causing all the pain. The hemorrhoid itself didnt hurt, the white thing sure did. Im pretty sure the white thing was not caused by any of the meds i took though. What is it? I have searched the internet like crazy but so far, no one has had a similar painful white (transluscent) lump on an external hemorrhoid.  On top of that, the reason why i finally plucked up courage to ask this online is the overwhelming concern that I might be constipated again. I am still following the super perfect diet, my body is literally a temple at this point but my poop is still normal/hard. It isnt soft like good quality poop is. This has been going on for 2 weeks and not much progress in poop condition at all. Everytime it still hurts like crazy. These few weeks, i have been pooping twice per day. But starting yesterday, i have not had any urge to poop at all. I am extremely worried that when i really poop, the hard feaces will make the pain drive me insane. As far as i know, i dont have any reliance to stool softeners etc. And i am just extremely worried and confused about whats happening. I also have ansolutely no idea how i am going to heal this hemorrhoid. I have tried every method suggested by the doctor and by the internet (i.e. sitz bath, good diet, not straining, meds etc)but its not working that well. I really dont want to opt for surgery because of certain family reasons. Pls help me. Thank you
