Abnormal ultrasound pls give some advice

Hi everybody.
I'm new in this site and I'm hbsag+ for more than 20 years possibly since birth.37 yrs old hbeag negative antihbe negative. I had a biopsy in 2008 and found Fibrosis 1 normal alt ast no treatment. After that my alt/ast levels started to elevate between 50- 100 but my dna was very low between 50-200 ıu/ml. The doctor said the elevated alt ast wasn't related to hbv since dna was always very low.He told me to lose weight.But the alt ast was never normal. Further tests and otoimmun markers were no problem. Another biopsy was done in July 2013. Before biopsy ultrasound was normal. Fibrosis was found 3/6 prescribed lamivudine since 2013.We dont have fibroscan in our city. the treatment was good my alt ast decreased to normal levels.But in  June 2015 there was some problem with the ultrasound. It said slightly coarsened parenchyma. The other details was fine. Last month another ultrasound was done. This time slightly heterogenous parenchyma possibly secondary to hepatitis b. The other details were fine again. The ultrasound doctor said  my parenchyma was just slightly heterogenous but there was no sign of cirrhosis. I talked to my doctor about the abnormal ultrasound results but he didn't even consider about it at all he told me ultrasound is similarly to fortune telling. He put me on viread. I'm so anxious although ultrasound doctor said no sign of cirrhosis. How can you evaluate my situation? Can it possibly be earlier stage of cirrhosis?

Ask for an MRI and see what the results are. In these situations, the doctors should be doing more than an ultrasound to rule out anything serious. Thankfully, my ultrasound for my liver comes back completely normal.. though it does find small cysts on kidneys and gallbladder when I need to keep an eye on. Keep up with the diet, weight loss, no alcohol and of course the medication.

use tenofovir, why use weaker  and more expensive drugs?

tenofovir lowers hbsag to less than 1000iu/ml by 5 years on half of treated patients (less than 100iu/ml about 20%) why use etv if effect on hbsag is weaker?

What is your AFP? Mine always come 4.

Mine is always between 2-3. As far as I know afp is much more related to hcc but not cirrhosis.

this means nothing liver is ok but lamivudine is not a drug for hbv anymore, who the crazy doctor prescribing such dangerous drug which just muattes hbv to worse?tenofovir or entecavir are the only antiviral for hbv, now for you only tenofovir because after using lamivudine all other antivirals can tbe used

The medical treatment for hbv was very frustrating in my country for a long time till the middle 2015. You had to have at least 2/6 fibrosis or 7/18 hai index to get any treatment.In case of treatment since my dna was low I was only prescribed lamivudine. Since last summer (I'm not so sure) there are no limitations as my doctor told. He finally put me on viread for one month. He told me that there wasn't any resistance to lamuvidıne.
Stef I wanna ask something. As I didn't resist against lamivudine do I have the chance to use entecavir if I need it sometime later or not. Because as far as I know and you stated above if there is resistance to lam it seems to possibly be resistance to entecavir. I'll be glad if you inform me about this. Thanks for your good response...

Thank you so much for your response. My doctor is a prof as well.My afp is also in normal range all the time. My next US is 6 months later. Hope it will be fine...Wish everybody good results...

Well my first ultrasound was in 2011 and since then it always read mild coarsened heterogeneous liver and my professor really dont pay much attention to it.
